Receiver #4 stopped working

Latest Ubuntu Disco Dingo
It worked fine for about 30 min, then something happened I had to pull the battery out to reset the machine.
Rtl_test recognizes all 3 receivers and the LED turns on when flip the 4th switch but nothing happens, only 3 devices are recognized.
I ran the rtl_test in Windows, I can confirm only 3 receivers are turning one by one.

You do start testing from device zero and not from device number one ?

I did not use -d 0. Just turned off all switches then turned on every switch in sequence. When I turn on switch one I see one receiver, when I switch one and two I see 2 receivers, when I switch one, two and three I see 3 receivers but when I switch all 4 on, I still see only 3 receivers.

So if you turn off receivers 0,1,2, and only leave receiver 3 on, does it work? Can you test all receivers 1 by 1 (turning all off except for 1?). That will confirm if one unit has died or not.

How were you powering the unit? Via a single cable, or with a separate power supply? The other thing I can think of is that if you were powering it all from your PC, maybe the thermal fuse tripped because your PC can't put out enough power. If you wait a few hours it should reset back to normal.

If I leave just receiver 3 on, rtl_test can't find any devices. I tried turning every switch one by one and all work except #4.
If I turn on all switches only get receivers 0,1,2. There used to be 0,1,2,3. So I'm assuming #4 is not working.
I tried plugging an additional 2.1A amp supply via USB C, same thing.

It worked at first with my Ubuntu laptop. I was able to get samples from all 4 receivers. Then it stopped working.
I tried rtl_test on Windows and Linux with the same results. At first, Windows complained about a device reporting a problem, then not even that.
